What You Need to Know Before Your Visit

Insurance Referrals and Requirements

Some insurance plans require a Primary Care Provider (PCP) referral to see a specialist. It is the patient’s responsibility to obtain this referral before scheduling an appointment. Once our office receives your referral, we’ll contact you to arrange your visit.

Referral Details:

• Referrals typically include a valid from date and expiration date or a limited number of visits.
• Our office will notify you if your referral is nearing expiration before your next appointment.
• If a renewal is needed, the patient must request an updated referral from their PCP.
